The consumer credit notification form is provided in Adobe Acrobat's portable document format (PDF).  If you have not downloaded and installed the Acrobat Reader you will not be able to view or print the Consumer Credit Notification form.

Filing instructions are included on the notification form.  If you wish to engage in Consumer Credit  activities with Utah Citizens, complete the form and return it with the appropriate fee.
